Fill out the New Customer Form - This will help set up your preferences for the computer system.
	Fill out the Credit Application for New Customers or Send us the usual Credit References: Banking, Corporate Information (you may send in your own form if available)
	Terms are 1% 10 Days – Net 30 or pay by Credit Card (See Below) or Wire Transfer (Email Us for information).

All three major credit cards below are accepted.

Call Accounts Receivable (216.641.3290) with the following information:

• Card number
• Expiration date
• Name on the card

Credit Card Preferences form and send it back attention Accounts Receivable.
